What products can I use to soundproof between floors in my house?

I am looking to acoustically and ideally thermally insulate between the floors of a house I am renovating. The floor/ ceiling area is approx 40m2 and has an available depth of approx 8" except where pipes and wires are run which may reduce it to 4" in places. I would also like to acoustically proof the internal walls upstairs which are yet to be built and therefore have no predetermined thickness but will have a total area of approx 50m2.

Please can you advise on the best products for the jobs, It will be used as a normal house but with variable bedtimes etc. NOT a recording studio or similar. Cost is an issue, but not a decider as I intend to live in this house for at least the next 30 years. Approximate costs per square metre for your recomendations would help me.

I will be installing myself, and as a professional builder have experience of most things. All Floorboards will be lifted anyway so solid foam products would be fine to install, Thermal insulation would be a bonus.
